What paperwork do I need to fill out?
You need to fill out the Student Agreement Form and the Assumption of Risk Form
then get it signed by both your faculty advisor and community site supervisor.
You can go to The Center for Service-Learning in the Kirk Center to pick up
these forms or download them here:
Please remember to inform us of any changes in address or telephone number as you do your Service-Learning project.
How do I register?
The last step is to turn in your completed and signed forms to
The Center for Service-Learning in the Kirk Center. The Service-Learning staff
will finish the registration process for you.
Please do
not register for a Service-Learning class anywhere else but in The Center for
Service-Learning!
