Application Checklist

We appreciate your interest in the Dental Hygiene Program at Mesa Community College. Please follow this checklist carefully to ensure that your application and documentation are complete. All of the following items must be postmarked by the due date in order for your application to be considered. 1. Mesa Community College Student Information Form, if not currently enrolled.
   
2. Dental Hygiene Application- Parts I through VI. (pages following)
   
3. Dental Field Employment/Observation form. This form must be submitted even if applicant has no prior dental field employment or observation experience.
   
4. Employer Verification of Dental Field Employment/Observation.
   
5. Reference Form Information sheet. This form must be signed by the applicant and returned along with the Dental Hygiene Application.
   
6. Reference Form completed by employer and mailed directly to Office of Admissions.
   
7. Official transcripts from all colleges and universities attended. These must be sent directly from the institution to the Office of Admissions.
   
8. Copy of current CPR Healthcare Provider card. Please provide a copy of both sides of card.

9. If claiming exemption from CRE101, attach documentation of ASSET, COMPASS or ACCUPLACER scores to application.
   
10. Copy of current certificate for Certified Dental Assistant.

All selected applicants will be notified by the Selection Committee of their acceptance status no later than April 1, 2010.
