Special Instructions:

The student agrees to the following guidelines:

1. I will submit an interpreter request form for each time that I need to have an interpreter for special assignments. This is other than for your regular interpreter in your regularly scheduled classes.
2. Each interpreter request form that I submit will be complete, signed, and dated.
3. Each form completed must be submitted to the DRS office a minimum of two (2) working days prior to each event of test date. The exam date and weekends do not count.
4. In the event of a change in the test date of time, I accept responsibility for notifying the DRS office of the change by altering and initialing this form a minimum of two (2) working days prior to the event or test date. An email submission is adequate if submitted to both Wink Harner, DRS Coordinator and Lisa Hitzler, Coordinator of Interpreting Services.
5. If submitted online actual signatures are not required.
