Verification Forms

Dependent Verification Form

If you had to include parent income information on your FAFSA and you were selected for verification use the Dependent Verification Form (DV Form).

Make sure that both you and your parent(s):

Complete all parts of the DV form

Sign the DV form

Complete worksheets A,B, & C

Include a typed and signed copy of 2006 Federal Taxes (if required to file)

That all tax forms are signed

To include all 2006 W2's

To bring in copies of other income sourse (if applicable) such at SSA1099

Independent Verification Form

If you did not have to include parent information on your FAFSA and you were selected for verification, use the Independent Verification Form (IV Form)

Make sure that you:

Complete all parts of the IV form

Sign the IV form

Complete worksheets A,B, & C

Include a typed and signed copy of 2006 Federal Taxes (if required to file)

That all tax forms are signed

To include all 2006 W2's

To bring in copies of other income sourse (if applicable) such at SSA1099
